Pila ka adlaw nalang ipahigayon na sab sa Lokal nga Panggamhanan sa Butuan pinaagi sa City Civil Registry Department ang 𝗙𝗿𝗲𝗲 𝗕𝗶𝗿𝘁𝗵 𝗥𝗲𝗴𝗶𝘀𝘁𝗿𝗮𝘁𝗶𝗼𝗻 sa tibouk bulan sa Pebrero.
Kung wala pay Birth Certificate, i-andam na ang mga rekisitos nga gikinahanglan aron makabenepisyo sa libreng pagparehistro.

Ania ang mga rekisitos nga dalhon:
- PSA Negative Certification
- LCR Verification Form (if child is 31 days old and above; subject for 10 days mandatory posting)
- Affidavit of Two Disinterested Persons (who might have witnessed or known the birth of the child)
- Registered Marriage Contract of Parents (if applicable; photo certified by LCR, present the original)
- Any two (2) proof Birth:
A. Baptismal Certificate or Certificate of Akeca
B. School Record (Form 137-E)
C. Immunization Record with Doctor or Midwife's signature over printed name, designation, and license number
D. Voter Certification
E. PhilHealth Membership Data Record
F. SSS Membership Data Record (parents or owner document)
G. Driver's License or Professional License or Passport
H. Income Tax Return (Parents or Owner Documents)
I. Barangay Certification of Facts of Birth (Prepared by Barangay Secretary and Certified by Barangay Captain)
- Valid ID with Signature
- Registered Marriage Contact of the Applicant (18 years old and above; if applicable)
- Barangay Certification of Deceased/Death, Certificate of Attendant of Birth (Hilot or Midwife)
- Barangay Certification for birth assisted by Hilot (Applicable to births delivered on or after Augus 19, 2014; indicating the reason why birth is being assisted by hilot as this is not allowed under SP Ordinance No. 542-2014)
- 10 Days Mandatory Training
- Barangay Certificate (Proof of residency and indigency)
For Indigenous People (IP's):
- Tribal Certification
- Valid ID of Barangay Captain, Barangay Secretary and Tribal Chieftain
If Child is illegitimate at Birth:
- Personal Appearance if father and mother to sign the Legal Documents
- Valid ID with signature of Both Parents
Additional Requirement/s if a father or a mother is of foreign national
- Passport
- Birth Certificate of both parents
- Affidavit to use the Surname of the Father
*If Father is Currently not in the Philippines
- Certificate of Registration
- Affidavit of Acknowledgement of Paternity
- To file on the nearest Philippine Consulate General
- Sworn statement with Apostille
